Person Centered Planning Programs

Your life. Your priorities. Your path forward.

Person-Centered Planning starts with what matters most to the individual—their strengths, relationships, interests, goals, and vision for the future.

Our programs help individuals, families, professionals, and support teams explore possibilities, strengthen connections, and turn ideas into meaningful next steps. Whether someone is learning the planning process, building a broader vision for their life, or developing a formal service plan, each experience is designed around the person at the center.

Person-Centered Planning 101

Learn the process by experiencing it.

Person-Centered Planning 101 introduces participants to the principles, tools, and facilitation techniques used to create meaningful person-centered plans.

Rather than simply learning the concepts, participants experience the process firsthand. Through guided activities and interactive practice, they explore how thoughtful questions, creative planning tools, and meaningful conversation can help uncover what is important to a person and what possibilities may lie ahead.

Participants also have the opportunity to experience being the focus person within a planning session, offering a deeper understanding of how the process feels from the perspective of the individual at the center.

Participants will explore:

  • Core principles of Person-Centered Planning

  • The role of the focus person

  • Facilitation skills and techniques

  • Creative planning tools

  • Identifying strengths, interests, relationships, and goals

  • Turning ideas into meaningful next steps

  • Applying person-centered practices in everyday life and professional settings

Building New Worlds

Imagine what is possible—and begin creating it.

Building New Worlds takes Person-Centered Planning beyond a single plan and explores how people can build richer, more connected lives within their communities.

Participants are encouraged to think creatively about relationships, interests, opportunities, community connections, and the ways they want to contribute. The program helps individuals identify what is already working, discover new possibilities, and develop practical steps toward a life that reflects what matters most to them.

The focus is not simply on services or supports, but on building a life filled with meaningful relationships, experiences, and opportunities to belong.

Participants will explore:

  • Personal strengths, interests, and aspirations

  • Meaningful relationships and natural supports

  • Community participation and connection

  • Opportunities to contribute and share talents

  • Identifying new possibilities

  • Turning goals into achievable next steps

  • Building a life around what matters most

Self-Directed Plan Writing

Turning your vision into a plan.

Creating an Individualized Service Plan can feel overwhelming. Our Self-Directed Plan Writing service provides personalized support throughout the planning and writing process.

A dedicated plan writer works one-on-one with the individual and the people they choose to involve, using a Person-Centered Planning approach to understand their priorities, goals, relationships, support needs, and vision for the future.

Together, we help organize those ideas into a thoughtful, comprehensive plan that reflects the individual—not simply a list of services.

Plan writing may include support with:

  • Identifying personal goals and priorities

  • Exploring available supports and resources

  • Identifying community connections

  • Organizing information for the Individualized Service Plan

  • Connecting goals with meaningful supports

  • Clarifying next steps

  • Developing a plan that reflects the person's voice and vision

Every Plan Begins With a Person

Person-Centered Planning is not about fitting someone into an existing path. It is about discovering their path.

Through thoughtful conversation, creative planning, and meaningful connections, we help individuals explore where they are, imagine where they want to go, and identify the people, opportunities, and supports that can help them move forward.
